Setup My Account as a Patient

If your birth date is incorrect, you can go to your profile and make the change in your demographics.

When entering a new Password, the Password must have a Complexity score of 3 green dots or better.

Enter Date of Birth
Enter a new Password and Confirm Password (The higher the complexity the better)
Click SUBMIT

This includes the consent to use telemedicine services

1. Navigate to MY HEALTHCARE

2. Click on HEALTH FORMS

If you would like to complete your health history, please go to Complete My Health Forms as a Patient
