Use your Group/Practice TIN and NPI numbers
1. Input your TIN into the the first field
2. Input your NPI number into the second field
2. Click on 'LET'S GET STARTED.'
2. Read Instructions and Click on Continue
3. Input Information to Register
Note: Completion of all red asterisks are required.
4. Input your Billing Information
Note: Completion of all red asterisks are required.
5. Choose Desired Level of Membership
6. Select Providers to Include into your Group
1. Select Providers to add in your group
2. Input provider's email
3. Click on 'SUBMIT'
